If you cannot personally cast your ballot, Ontario’s guide says you may be able to appoint another eligible voter to vote on your behalf. That person is called a voting proxy.

The required form

Complete two copies of the official Appointment for Voting Proxy Form (Form 3). Both need the voter’s original signature; signing one and photocopying it is not sufficient.

Choose the proxy before signing

You must identify the proxy when completing the form. The person must be eligible to vote and should be someone you trust to follow your voting instructions.

Dates and local availability

Appointments can begin after nominations are certified. The guide says this will be done by 4 p.m. on August 24, 2026 in most municipalities. Proxy voting may not be available where mail, telephone or internet voting is offered. Confirm availability and deadlines with your clerk.
